Tracy I got a message from the people that has Dolly please send me your email so I can forward you the form my email is in your messages;

Hello,
Thank you so much for your interest in Dolly. I would love for you to have her as a part of your family. Obviously I know that you love dogs if you have 5 of them!!! LOL. I now have 5 dogs of my own as well because I have kept 3 rescue dogs I took in…just couldn't let them go. I do have one family interested in her that lives here locally…so I do have to meet with them first and depending on my impression of them we will go from there. If I do end up bringing Dolly out to you…I am heading down to Tennessee next weekend…so do you think you could meet me somewhere along the way? She is FABULOUS around other dogs and that's why I'm a little hesitatnt about the family that is interested in her locally because they don't have any other dogs and I really think she would do best with other dogs because she is so used to being with other dogs and I think she will be lonely without at least 1 k-9 companion. If you could fill out the attached application and we can go from there….thank you again for your interest.
